Also, if you take the time to mouse-over any of the medals, a little pop-up will tell you what they are for.
If you've played the map, the medal makes sense. The white wall has a hole in the middle, and the red wall often comes behind it (red has tiny holes on the sides). The medal is sort of a joke because when you see that combination, it usually means you are going to die.
The spikes represent the metal objects that come from the right to make the game even harder. Some of them are spikes.
