I'm too lazy to make you a screen shot, but all you need is a trigger_teleport and a info_teleport_destination.
The trigger_teleport is made out of a brush (or brushes) and the info_teleport_destination is a point entity. Look at their properties because the setup is pretty much self-explanatory.
