teleports in Hammer Mapping

how the hell do i add teleports that will teleport me somewhere ?

Ladder in Hammer Mapping

why when i add ladder and press alt + enter and chose ladder, save compile and run it, the ladder is invisible

Doors! in Hammer Mapping

well i built my school lol, and i added doors, butthe problem is: when i move close to door, it just fly away from me, i just want it to be opened like in cs_assault etc..
now it just fly away.............. and then it come back after 5-6 seconds.

how i fix this :p ?
