Box
A box can be made using a regular brush. If you want it to be breakable, select it, hit Ctrl+T, and select func_breakable from the list.
Door
You might find this to be complicated, but it isn't, just follow the instructions here. There used to be a picture of what the door looks like, but it's gone, so I'll try to find a picture for you.
Window
A window is a hole is your wall, nothing to it.
